Reports and Forms Authorization

1. Reports and Forms authorized for use within the district are those reports and forms reviewed by the Reports and Forms Review Committee and designated as approved in the Official Forms Index maintained in the Records Management Center.

2. Before printing or distributing any existing, new, revised or consolidated form for use in the district, the originating office must submit such form to the Reports and Forms Review Committee for review and inclusion in the Official Forms Index in accordance with the procedures set forth in the Forms Management Handbook.

3. A copy of any form listed in the Official Forms Index may be obtained by the public without cost within a reasonable time after submission of a written request to the Records Management Center.
